HOW TO REGISTER

You may register by:

  1. Visiting any Ministry of Labour and Social Security office (MLSS) in your parish. (Ensure that you collect a registration form and medical form).
  2. Visiting the JCPD website (Download the registration form and the medical form).
  3. You also have the option of visiting our head office located at 10 Ripon Road, Kingston 5. (The JCPD receptionist at the front desk will assist you).

Important points to note when applying:

  1. All applicants are required to participate in an Interview.
  2. Both registration and medical forms must be completed before you can participate in the interview, which is done by a Client Care Representative. This means you have collected both your registration and medical forms from the JCPD or any MLSS location and have completed and submitted both.
  3. The medical form is needed before you can participate in an interview. It must be completed by a Medical Doctor or Health Care Professional who is competent to make an assessment.
  4. If a parent, guardian or a caregiver is applying for registration on behalf of the applicant, but the applicant is able to write, the applicant MUST sign and date the form.
  5. If the parent, guardian or caregiver signs on behalf of the applicant, then the form MUST be certified by a Senior Social Worker, JP, Principal or a Medical Doctor

Required documents for your Registration and Interview process:

  1. Original copy of birth certificate
  2. Original copy of marriage certificate (If applicable)
  3. Government issued ID (any of the following-Passport, National ID, Driver’s License, etc.)
  4. Tax Registration Number (TRN)
  5. Two (2) certified passport-sized photographs
  6. National Insurance Scheme (NIS Card)
  7. Completed Medical Report Form
  8. Completed Registration Form
